REGISTRATION INFORMATION FOR 2004 WORKSHOPS

To register for a Sawtooth Science Institute workshop:

  • Obtain the three-part Non-degree Seeking Registration form from ISU Academic Outreach, 282-4545; the Sawtooth Science Institute, 788-9686; or the ISU registration office 282-3900.
  • Fill out the form and send it with your check to Idaho State University, Academic Outreach, Campus Box 8015, Pocatello, Idaho 83209. Please write your social security number on your check and your check number at the bottom of the form. Questions? Call SSI or ISU's Academic Outreach office at (208) 282-4545.
  • Keep the pink form; it is your receipt. Please find the index number and section for the course you are taking from the list on the back of this form.

If you wish to have upper division credit you must pay the full transcript fee. Please make note of this on the registration form.

  • If the course is 497/597 and you wish graduate credit, the purple form Professional Development form must accompany the registration form.

If you are over 60, please include a copy of your driver's license in order to receive the senior citizen rate of $20 to register and $5 per credit hour.

ALL STUDENTS MUST BE ENROLLED (ISU REGISTRATION FORM AND ALL FEES PAID) PRIOR TO THE MEETING DAY OF THE WORKSHOP -- you will not be able to register on site. Your instructor has been instructed to dismiss anyone not on the enrollment list.

COURSE REQUIREMENTS FOR THOSE SEEKING CREDIT:

  • Your enthusiastic participation in the full two days work (or four days, as the case may be) during the workshop,
  • The completed evaluation form and
  • The fulfillment of a short assignment or an exam (your instructors will provide more information). You may elect to prepare a short lesson plan that is based on what you have learned in this workshop. Please try to adapt it to your particular area; a lesson plan that will be useful in your teaching assignment is preferred.
  • The completed assignment is due two weeks after the workshop concludes.
